Examples of Health Action Plans
In this section you can see examples of Health Action Plans that can be created for people with learning disabilities.
The examples will be made up, but based on experiences of Community Nurses who have worked with people with learning disabilities to develop Health Action Plans.
There is just one example created so far – a plan for Sarah Jones.
You can see the Health Action plan written with Sarah and examples of the Easy read resources created to make things more accessible for her
